Home Infusion Nursing Coordinator - Permanent - Portland, Maine
Are you ready to transform how patients receive life-changing infusion treatments by bringing specialist care directly into their homes? Proclinical is seeking a Home Infusion Nursing Coordinator to support the implementation and ongoing success of a home-based infusion program across the United States. This role will assist specialist care teams through program adoption, stakeholder engagement, training, and process improvement initiatives.
Primary Responsibilities:
The successful candidate will help identify and resolve challenges throughout the patient journey while ensuring a positive experience for patients and healthcare professionals. In this role, you will:
- Conduct nurse-led patient assessments to determine suitability for home-based treatment in collaboration with physicians and multidisciplinary care teams.
- Develop strong partnerships with clinical and operational stakeholders, including healthcare providers, pharmacy teams, nursing teams, patient support services, and reimbursement teams.
Skills & Requirements:
- Graduate of an accredited school of nursing; BSN preferred.
- Current Registered Nurse license in good standing within the applicable state.
- Previous nursing experience in oncology care preferred.
- ONS Provider Card required within 12 weeks of hire; OCN certification is advantageous.
- Excellent written and verbal communication skills, with the ability to explain information clearly and sensitively.
- Strong collaboration and relationship-building abilities.
- Empathy and emotional intelligence, with an understanding of how change impacts healthcare professionals, patients, and their families.
- Strong problem-solving skills and a commitment to improving patient experiences.
- Self-motivated with a sense of urgency and accountability.
Advantages:
- Previous specialized home health experience.
- Formal training or certification in change management or business administration (e.g., Prosci).
Additional Responsibilities:
- Conduct and support patient screening, eligibility assessments, onboarding activities, and appointment scheduling within the electronic medical record (EMR) system.
- Ensure all clinical and operational requirements are completed prior to medication collection by home infusion clinicians.
- Support timely post-treatment communication and completion of EMR documentation.
- Collaborate with clinical, operational, and commercial teams to ensure successful program implementation and ongoing operational excellence.
- Monitor the patient onboarding pipeline and proactively address process bottlenecks to maximize service capacity.
- Identify, manage, and escalate risks, barriers, and opportunities affecting program delivery.
- Deliver training, education, and ongoing support to clinical teams to encourage adoption of program workflows.
- Drive continuous improvement initiatives to enhance processes, stakeholder experience, and program performance.
- Present program performance updates and reporting during regular stakeholder meetings.
