Your Main Tasks
- Collaborate in providing professional and individualized care and support for residents or clients.
- Recognize health status changes early, initiate appropriate measures, and adhere to internal standards and guidelines.
- Take responsibility for the care process, ensuring smooth workflows in your area of assignment.
- Document care processes meticulously and comprehensively.
- Conduct communications with doctors, visits, or discussions with specialist departments, and implement medical orders professionally.
- Engage in interdisciplinary work with other departments such as physiotherapy, activation, and social services.
- Supervise and support trainees while actively contributing to the ongoing enhancement of care quality.
What You Bring
- Completed training as a certified nursing specialist HF/FH or health professional EFZ.
- At least 1 year of professional experience in long-term, acute, or home care.
- Experience in handling daily responsibilities is an advantage.
- A passion for interdisciplinary collaboration with doctors, therapists, and other professionals.
- An independent, responsible, and empathetic working style.
- Proficient German language skills, both spoken and written.
