Job Description

To primarily provide holistic, evidence-based best practice care to a private individual relating to all her needs in a positive person-centred manner. These needs currently include having a diagnosis of dementia, age-related frailties, and any other health co-morbidities that may arise. The care provision may potentially include 2 clients.

  • Assess, plan, develop, and implement care programs; provide advice; maintain associated records.
  • Carry out nursing procedures in accordance with clinical guidelines.
  • Provide clinical supervision to other staff.

Person Specification Criteria

Qualifications & Training:

  • ESSENTIAL: Educated to SVQ level III or equivalent.
  • DESIRABLE: Additional qualifications that benefit LHS such as Moving and Handling Trainer, Beauty/Massage Therapy.

Language

ESSENTIAL: The ability to communicate effectively using English. English as a first language.

Experience

ESSENTIAL: Minimum 5 years of experience working with people with dementia.

Skills and Abilities

  • Demonstrate excellent team working skills and the ability to work using own initiative.
  • Effective listening, communication, and interpersonal skills.
  • Excellent written communication in English.

Specific Job Requirements

Environmental conditions, unsociable hours, car driver, etc:

  • Commit to working a shift pattern based in Buckinghamshire, UK and Switzerland for extended periods.
  • Unsociable hours, including a sleepover arrangement alternating nights with another staff member.
  • Travel to and from places of work as required.

Scope and Range

  • Provide high-quality, safe, and supportive care in the family home or external environment.
  • Accountable for care standards within the context of clinical governance.

Main Duties/Responsibilities

Assess, plan, develop, implement, and evaluate care programs. Maintain effective communication with clients, relatives, and the multidisciplinary team.

Work within UK national and local policies, adhere to defined organizational procedures and standards.

Lead staff, manage patient records, and maintain patient belongings and items required for care.

Develop role through evidence-based practice, continuous improvement, and ongoing CPD.

Promote effective care, social engagement, respect for family and others, decision-making, and leadership.

Be a visible resource for clients and family, acting as a link for other professionals.

Teach staff and participate in staff development plans.
